Be allowed by a nonadministered facility. But the discussion of the auction facility envisioned that it would only supplement ongoing open market operations so that such operations would continue to provide the needed daily adjustments in the Federal Reserve's balance sheet. The Auction Credit Facility Within the context of an interest rate targeting strategy, the auction facility alternative would allow the Federal Reserve to adjust the size of its balance sheet to help hit a federal funds rate target because 1 ; depository institutions would bid for fixed amounts of Federal Reserve advances being offered in the auctions and 2 ; borrowers would not be permitted to prepay the advances. The Federal Reserve could ensure that it has control over these balance sheet adjustments because it could vary the amount of advances being auctioned. As discussed in the main text of this chapter, the Federal Reserve could vary the size of its auctions to account for secular, cyclical, and seasonal factors in the context of achieving the target federal funds rate. If the Federal Reserve had to extend a large amount of emergency credit, the maturing auction facility loans could help offset that emergency lending and thereby help maintain the target federal funds rate. In addition, emergency lending could be offset by reducing the size of subsequent auctions. The speed and effectiveness with which adjustment could occur would depend on the frequency of the auctions and the terms to maturity of the advances auctioned. If the emergency lending occurred between the dates of the auctions, the federal funds rate might fall below the FOMC's target until the next auction, when a smaller amount could be offered to offset the emergency credit extended. For the Federal Reserve to control its balance sheet in such a case would require that some part of auction facility advances be overnight loans, that some auctions be conducted daily, or that the Federal Reserve retain a sufficient portfolio of System Open Market Account SOMA ; assets whether Treasury or non-Treasury securities ; that it could sell to offset an increase in emergency lending. It was the last optionthat auctions would supplement open market operations, not supplant themthat was incorporated in the auction facility discussion in the main text of this chapter. Consequently, if the Federal Reserve extended a substantial amount of emergency loans, adjustments to its balance sheet would not be made solely through the auction facility. In sum, auctions of advances appear to be broadly consistent with the principle of maintaining the central bank's instrument independence within the context of using an interest rate targeting strategy. The Nonadministered Credit Facility Under the nonadministered credit facility, depository institutions would obtain overnight advances at the target federal funds rate. The amount of advances from the nonadministered facility that would be on the Federal Reserve's balance sheet would be demand-determined and based on the FOMC's target for the federal funds rate.1 The FOMC would control the amount borrowed from the nonadministered facility through its control over the federal funds rate target. For a summary of the Overall Clinical Safety, see above. EXPERT REPORTS A satisfactory Clinical Overview has been submitted by a relevant qualified person. The review of the data for the finished product, both with and without benzalkonium chloride is satisfactory. SUMMARY OF PRODUCT CHARACTERISTICS This is identical to the SPC for Oftaquix 5 mg ml Eye Drops, albeit with any statements referring to benzalkonium chloride removed. This is satisfactory. PATIENT INFORMATION LEAFLET This is identical to the UK approved PIL for Oftaquix 5 mg ml Eye Drops with preservative, albeit with any statements referring to benzalkonium chloride removed. This is satisfactory. LABELLING This is identical to the approved labelling for Oftaquix 5 mg ml Eye Drops with preservative , albeit with any statements referring to benzalkonium chloride removed. This is satisfactory. MAA This is clinically satisfactory DISCUSSION This dossier for a standard abridged application of Oftaquix Unit Dose 5 mg ml Eye Drops without preservative demonstrated that the formulation is otherwise the same as the preserved Oftaquix 5 mg ml Eye Drops, albeit with the preservative benzalkonium chloride removed. The removal of the preservative is compensated for by the single-dose container packaging of the solution. In addition, the packaging in single-dose container may increase compliance. However, compliance data on the unpreserved unit dose formulation has not been submitted so this claim can not be substantiated. Proof of improved compliance in clinical studies should be presented before any claims of improvement with the unit dose are made. It has been shown that the antibacterial properties of the finished product without the preservative are the same as the formulation with the preservative for treating the three most commonly occurring ocular pathogens Haemophilus influenzae, Staphylococcus aureus, and Streptococcus pneumoniae ; . No clinical efficacy studies have been provided. This is satisfactory as the applicant has already shown through the in vitro and pharmacokinetic studies performed that Oftaquix Unit Dose 5 mg ml Eye Drops has the same antibacterial activity as Oftaquix 5 mg ml Eye Drops, which has already been granted a UK licence and undergone a first-wave recognition.
Reflecting standard oncology practice, which often requires uses and dosages for therapeutic products that have not been approved by the US Food and Drug Administration FDA ; , this educational activity includes information about many drugs. All faculty participating in continuing education activities are expected to disclose the approved or investigational status related to the subject matter of this activity--supportive care in cancer ; of all products and devices under discussion. This information, as of the time of printing, is briefly summarized below. In addition, primary references and ultram. Peak, for single compartment locations from 1 to 6; this factor becomes progressively smaller for input locations 8 and 10, but it is more than 4.6 for the case of uniform input to all compartments. Rising slope divided by peak. The slope, du dT, is determined at the point where the rising EPSP attains half of its peak amplitude; this slope is often the maximal rising slope. An amplitude independent quantity is obtained by dividing this slope by the EPSP peak amplitude. The resulting dimensionless shape index values cover a sixfold range, from 2.5 to 15.5 in Table 1. Thus, for a membrane time constant, 7 5 msec, the quanti ty, dvN w lv 10 alone, to 3.1 msec-l ranges from 0.5 msec-' for input to compartment for input to compartment 1 alone. There is an approximate inverse proportionality between these rising slope peak values and the time-to-peak values, in other words, the time to peak is approximately 1.6 times the reciprocal of the rising slope peak value. A similar proportionality was found to apply to many experimental EPSP shapes, and also to theoretical EPSP shapes computed with either the fast or the slow synaptic conductance time course. Falling slope divided by peak. Here the slope, dv dT, is determined at the point where the falling EPSP attains half of the EPSP peak amplitude and this slope value is divided by the peak amplitude. A value of -0.5 corresponds to uniform passive decay. The cases where synaptic input was confined to compartments 1, 2, 3, or 4 all fall more rapidly than this because of electrotonic spread * out into the peripheral half of the chain, while the cases where sypnaptic inpu t was confined to compartments 6, 8, or 10 fall slightly more slowly because of electrotonic spread toward the soma from the peripheral half of the chain. Plot of half-width versus time to peak. In comparing these theoretical EPSP shapes with EPSP shapes observed in motoneurons, it was found useful to represent each shape as a point in a two-dimensional plot: the ordinate of each point is the half-width value, while the abscissa is the corresponding time to peak. Several examples of such shape index plots are illustrated in a companion paper 14 ; . Such plots provide a means of grasping and comparing the variety of EPSP shapes found for different values of the theoretical parameters. Effects of fast, medium, and slow synaptic input transient. Table 2 provides a summary comparison of EPSP shapes ob tained in three different computational series, using the three cases, fast, medium ., and slow, of synaptic The numerical details of Table conductance time course defined in METHODS. 2 are presented with the hope that they may be found useful in the examination of experimental results from various neuron types having different membrane time-constant values, and a similar time course of synaptic current or synaptic conductance; one example of this for cat motoneurons ; is provided in a companion paper 14 ; . Here, comments are made about only a few general aspects of these numerical results. Perhaps most striking is the fact that changing the input time course at single compartmental locations has a proportionately much larger effect for proximal input loca. In urine for about 8 days and for up to 30 days in chronic users. State toxicology testing of law enforcement evidence shows that MDMA is the fifth most common drug detected at the Seattle lab n 44, 1.4 percent ; and the ninth most common for the rest of the State n 67, 0.5 percent ; exhibit 10 ; . The related compound MDA was detected less often in Seattle 10, 0.3 percent ; and the rest of the State 15, 0.1 percent ; . PCP was found in 29 samples 0.9 percent ; in Seattle, but it was not among the top 25 drugs for the rest of the State. Calls to the ADHL regarding MDMA continued to decrease substantially from 218 in 2001, to 104 in 2002, to 20 in the first half of 2003 for callers of all ages. LSD, not frequently mentioned in 2001 or 2002, was mentioned only once in the first half of 2003. Collectively, these drugs represented just 2 percent of all calls, compared with 6 percent in 2001. Doses of ecstasy average in King County and throughout much of the State. The U.S. Customs Service first provided data indicating seizures of MDMA in the first half of 2002. The number of seizures and amount of product seized, while never huge, has continually decreased over the three 6-month reporting periods. In the first 6 months of 2003, four seizures totaled 32 pounds, the largest of which was 28 pounds. Other Drugs of Note--Muscle Relaxants and TriCyclic Antidepressants Muscle Relaxants Muscle relaxants are a category of drug that is often overlooked in the investigation of drug abuse trends. In past reports these medications were categorized as "other drugs" and not discussed. These drugs can have potent sedating effects in addition to their impact on muscle tissue. Use of muscle relaxants in combination with other depressants such as alcohol or benzodiazepines is contraindicated. Key informants note that cyclobenzaprine e.g., Flexeril ; and carisoprodol e.g., Som ; are purchased on the street with the intent of using them to get high. Mortality data show a small but increasing presence of such substances in drug-involved deaths exhibit 16 ; . From 1997 to 2003, there were 30 deaths in which a muscle relaxant was identified, increasing from 2 to 11 during this timeframe. Cyclobenzaprine 16 mentions ; and carisoprodol 13 mentions ; were and tretinoin.

The lack of significant difference between the inhalant agents in terms of quality of induction was unexpected. Expected, however, was the improvement in quality provided by premedication, and by induction with propofol over the inhalant agents. The clinical environment, as discussed above Section 4.3 ; , assumes a major role rationalising these results. Variations in each dog's response to handling, intravenous catheter placement, placement of the induction mask, or other factors unknown, appear to assume greater importance than the variation between the three inhalants in terms of irritancy to the respiratory tract. Sevoflurane has previously been reported to give an improved quality of induction of anaesthesia over isoflurane in dogs Johnson et al. 1998 ; . It is suggested that this is due to sevoflurane's lower irritancy to the respiratory tract, which leads to lesser decreases in tidal volume and minute volume on inhalation Doi and Ikeda 1993 ; , combined with sevoflurane's lower blood: gas solubility allowing faster changes in concentrations at the level of the CNS. The lack of difference between any of the three inhalants in the present study therefore demands explanation. 4.4.2.1 Measuring quality All measures of quality of induction of anaesthesia are in essence subjective. The scale used to record this variable in this study has not been previously reported, although it is similar to that used by Johnson et al. 1998 ; in several of the behaviours on which the final score was based. Basing the score for quality of induction on several areas of behaviour exhibited by the animal provided more consistency to the scoring process. However, until the dog is able to converse on equal terms with the investigator to explain its experience of the process of induction of anaesthesia, all descriptions of quality of induction will be limited. More information on the animal's response to induction may have been provided by the use of invasive monitoring of variables such as arterial blood pressure and oxygenation from before the commencement of induction. However, requiring previous general anaesthesia or at least deep sedation, placement of such instrumentation would not have been possible within the design of the study.
